              GSSSI Presents "State Home Care"
Greater Springfield Senior Services State Home Care program is designed to assist older adults with the opportunity to continue living within their homes safely and independently. We offer a person-centered approach to care planning that meets the needs of each participant by providing helpful services such as grocery shopping, housekeeping, caregiver support and personal care. GSSSI’s State Home Care Program provides various levels of support to individuals who require assistance on an ongoing basis in order to keep them living safely and independently at home for as long as possible. 
GSSSI offers a variety of service supports including:
  • Case Management
  • Adult Day Programs
  • Assistance with Personal Care
  • Food Shopping
  • Housekeeping
  • Laundry
  • Meal Preparation
  • Meals-On-Wheels
  • Personal Emergency Response Systems
  • Transportation for Medical Appointments
To be eligible for GSSSI’s State Home Care program, interested individuals must be at least 60 years of age, or under the age of 60 with a diagnosis of Alzheimer’s Disease or a Related Dementia. Interested individuals must also have an assessed need for assistance with tasks such as bathing, dressing, housework, laundry, grocery shopping, medication reminders, meal preparation, or medical transportation.

Recipients of these services must have an annual gross income less than $28,865 for an individual; $40,842 for a couple (Note: income amounts change every January 1). Individuals whose incomes exceed these limits may be eligible for services through GSSSI’s Over-Income Program. For those experiencing financial difficulties, adjustments to the assessed copay may be available.

Specialized programs offered within the SHC program:

Enhanced Community Options (ECOP) and Community Choices (CHOICES) are options for individuals who become clinically eligible for a nursing home and require a high level of services to stay in their home. These programs may be available for both MassHealth and non-MassHealth consumers.

Alzheimer’s and Dementia Related Diseases (ADRD) Care Management is a program for consumers with complex dementia and are at a high risk for facility placement. Our Certified Dementia Practitioner will make care recommendations; help coordinate services and supports that assist families in caring for their love ones in the community.
Advocacy & Navigating Care in the Home with Ongoing Risks (ANCHOR) is a program that provides a higher level of care management that offers more rigorous and time intensive support for older adults with behavioral health needs who are may be at risk due to their inability to accept or retain in-home support services.
Consumer Directed Care is a service delivery model within the State Home Care Program that is modeled in part after the PCM Program. The CDC Program allows the consumer to train, hire, and supervise their own worker as opposed to having a worker employed by a home care agency.
